Mechanism: Transcutaneous (external) electrical spinal cord stimulation.
Status: FDA Cleared (2024). This non-invasive device is approved to improve hand and arm function in chronic tetraplegia. It is currently being deployed in select rehabilitation clinics in the US.

Neuralink (PRIME Study)

Mechanism: Fully implantable, wireless Brain-Computer Interface (BCI).
Status: Early human trials are active. Participants with quadriplegia have successfully controlled computer cursors and played video games using only neural activity. The study assesses the safety of the N1 Implant and R1 Robot.

NVG-291 (NervGen Pharma)

Mechanism: Peptide injection inhibiting the PTPσ receptor.
Status: The "Chronic" cohort has completed analysis, with top-line data released in late 2025. The "Subacute" cohort (20-90 days post-injury) is currently active and continuing dosing as of early 2026.

Supramolecular Polymers ("Dancing Molecules")

Mechanism: Synthetic nanofibers with high molecular motion developed at Northwestern University.
Status: FDA granted Orphan Drug Designation in July 2025. Animal models showed reversal of paralysis in 4 weeks. Human trials have not yet begun (Target: Late 2026).

Mechanism: Epidural spinal implant targeting the sympathetic nervous system.
Status: The "Empower BP" pivotal trial is actively recruiting. Unlike motor-focused implants, this specifically targets blood pressure regulation to treat Orthostatic Hypotension (fainting) in high-level injuries.

iPSC-Derived Neural Stem Cells (Keio Univ)

Mechanism: Transplantation of Induced Pluripotent Stem Cells (iPSCs) into the spinal cord.
Status: First-in-human study completed (April 2025). 4 patients received transplants; 2 showed motor score improvements. The team is now preparing for the next phase of clinical trials.

Autologous Engineered Spinal Cord (Tel Aviv Univ)

Mechanism: 3D-printed neuronal networks using the patient’s own cells embedded in hydrogel.
Status: Approved for "Compassionate Use" trials in 8 patients as of August 2025. Because the tissue is autologous (from the patient), it eliminates the risk of immune rejection.
